We know you have many choices when choosing a Dentist in

IN so we have made requesting an appointment a simple process via our Web site. If, for any reason you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible.
Insurance and Billing
Our office accepts a variety of different insurance carriers. Please call our office and our staff will help determine your dental benefit as estimated by your insurance carrier. We are not an Indiana Medicaid Provider.
Payment Options
- We accept cash, checks, Visa, Mastercard and Discover. There is a $30.00 service charge for returned checks.
- Patients receiving major restorative treatment (crowns, bridges, veneers, etc.): One half of the total fee is due at the initial (impression) visit. The remainder of the fee is due at the final visit. Insurance benefits may substitute for the initial fee.
- We ask all emergency and new patients to pay in full at the time of their initial visit.
- Financing can also be arranged for larger cases. This can be done through Care Credit or other third parties, and may be extended as long as 48 months. Credit check required.
- Some extended payment or interest free programs may be available on a case by case basis.
Minor patients accompanied by an adult
The adult accompanying a minor is fully responsible for payment of all services rendered. Any legal agreement for child support and/or payment for dental services is between the judge, the mother and the father.
Patients with dental insurance
Dental insurance is meant to reimburse a portion of the cost of your dental treatment. We are happy to complete the necessary claim forms, and to submit them on your behalf. However, the insurance is a contract between you, your employer and the insurance company. We are not a party to this contract. We will not become involved in any disputes between you and your insurance company regarding deductibles, coinsurances, covered charges, secondary insurance and/or "usual and customary" charges etc. other than to supply factual information as necessary.
